What happens if I lose my certificate or transcript?
Good news — we keep your records on file for 25 years. If you misplaced or lost a certificate or transcript from our school, we ask you to complete "Section One" in our Student Verification form. We can verify you as an Oregon Medical Training graduate through your graduation date and personal information listed in the document.
If approved, we will send you a $5.00 invoice per document requested in the SV form. For example, if you request both completion documents, you will be charged with a $10.00 invoice to the email you listed in the document. If you only ask for one document, you will be sent a $5.00 invoice.
